Released in 2006, this rare Exclusive Edition was limited to only 666 made and features the exclusive Gag Machete accessory which fits on the side of Jason's head to recreate his death scene at the end of the movie.

The box's display window is signed by Ted White who played Jason in Part 4 and has since passed away. The autograph was obtained at Monster Mania Con in 2017.
