A used Orginal GM 3 Speed Heater Switch with Control Cable (Non AC) that fits:

This belongs to a friend of mine.  It came off his 1964 Corvette.  This has been tested and works just fine.  5 wiring connectors.  Cable moves in and out smoothly.

NOTE:  This is for NON AC cars only.
