The fruit body of Hydnum repandum typically stands 5-10 cm tall. Its cap, ranging from 3 to 12 (or sometimes 15) cm in diameter, is fleshy, dry, and dense. It features a rough, uneven surface, often assuming irregular shapes. In its youth, the cap is slightly convex with curved edges and a slightly velvety texture. As the fungus matures, it flattens out with a concave middle and develops wavy, sometimes lobed edges. Often found growing alongside neighboring mushrooms, the cap does not separate from the rest of the fruiting body.

The color of the cap ranges from light ocher and pinkish yellow to reddish orange and light nut. When pressed or fully ripe, it darkens, taking on a reddish-orange hue. In dry weather, it fades to a pale yellow or whitish color.

The flesh of this mushroom is dense and fragile, appearing whitish or yellowish, and turning brownish-yellow when broken. It emits a faint, pleasant fruity smell. With age, the flesh hardens and acquires a bitter taste.

The hymenophore, or the spore-bearing surface, is comprised of frequent needle-like spines that descend along the stem, occasionally reaching the very base. In young specimens, the spines are short and resilient, appearing whitish to yellowish-pink. As the mushroom matures, the spines grow to be 4-8 mm long, matching the cap's color and easily breaking and flaking.

The stem measures 3-5 (and sometimes up to 8) cm in length and 1.5-4 cm in diameter. It is dense, solid (sometimes with cavities), cylindrical or slightly curved, often extending at the base, and occasionally eccentric concerning the cap or even central. The stem's surface is smooth (felt-like at the base), dry, and whitish or yellowish, darkening as the mushroom matures.

This species produces white spore powder. The spores measure 4-8.3 × 5.4–6.3 µm, appearing broadly ellipsoidal or ovate, smooth, colorless, and not amyloid.
