What a sweet vintage surprise!
Complete with the Needle Threader Attachment (with a threader hook installed), the setting gauge, 2 spare threader hooks, original instruction sheet and box.

Your Vintage Sewing Machine Needle Threader fits and operates on Singer sewing machines such as models 15, 27, 28, 66, 99, 101, 127, 128,185 and possibly other side attaching low shank straight stitch vintage sewing machines.
NOTE: The needles on these compatible sewing machines thread from LEFT to ----> RIGHT.

PLEASE NOTE: Unfortunately, this vintage automatic needle threader is NOT COMPATIBLE with sewing machines with needles which must be thread from RIGHT to LEFT such as the Singer model 201, Featherweight 221 or the 222k.
I am using my Singer Featherweight 221 to illustrate and demonstrate how to fit your sewing machine needle threader attachment and how to use it. I do not have a sewing machine with a left threading needle (yet).

This terrific needle threading aid will ONLY thread from LEFT to RIGHT. It will not fit or operate on slant needle models or sewing machines with a larger circumference (thicker) presser bar like the Singer 301-301a. See the thread cutters pictured below as an example. The thread cutter on the left is from my Singer 301a and the smaller thread cutter is from my Featherweight 221.

Archive pictures are used to illustrate and demonstrate the operation and fitting of this type of vintage automatic sewing machine needle threader.
So!
This is how your vintage Sewing Machine Needle Threader Attachment is attached to your left to right needle sewing machine.
Remove the presser foot and the thread cutter from the presser bar. The automatic sewing machine needle threader has a thread cutter built into the left side of the attachment.

With the presser bar lever up, slide the needle threader up the bare presser bar almost as high as it will go.

The built-in thread cutter is positioned on the left (black arrow)...

...and the triangular threader mechanism is on the right.

For the moment, tighten the little cuff screw (blue arrow) using your very small screwdriver to hold the automatic needle sewing machine needle threader in place while you prepare the setting gauge.

Turn the hand wheel of your sewing machine until the needle is in its highest position. Remove the machine needle and insert the setting gauge as high as it will go with the flat side to the right. Tighten the needle clamp just as you would if you were changing your sewing machine needle.

The notch should be facing right.

This next step is crucial to the exact fit. Swivel the triangular needle threader mechanism forward until the grooved tip fits into the notch of the setting gauge. You may need to loosen the little cuff screw to slide the automatic threader up or down the presser bar.

When the needle threader attachment is placed correctly, firmly tighten the cuff screw on the metal swivel cuff (red arrow). This secures the attachment to the presser bar.

Replace the presser foot and the machine needle.

With the presser bar lowered, raise the machine needle using the hand wheel until the needle is in its highest position.

The cuff has an inner rotating cylinder. When the threader is gently swiveled  to the front with a finger, the threader sits to the right side of the needle's eye.
Slowly press the sliding screw to the left until the slender threader hook passes smoothly through the needle eye (red arrows) and hook the thread.

Slowly release the sliding screw.
The thread will follow.

The threader then returns to the back right side and out of the way on its own.

If you wish, you can flip the threader up making it even more invisible!

Note: This a delightful vintage treasure and a wonderful tool for those who have difficulty threading a sewing machine needle. Be slow and gentle with the threader hooks. They are fragile.
Next!
Changing the automatic sewing machine needle threader hook.
The parts are tiny and easy to lose. Consider changing the threader hook over a light colored surface with good lighting. A magnifying glass is very useful.
Start by removing the screw used to slide the threader hook to the left. This little screw also holds the slim threader hook and an essential tiny wire in place.

See the tiny wire holding the threader? It is held in place under the round end of the threader hook. Take GREAT care. The wire can spring out and become lost during the threader hook change.

The new threader hook needs to go in hook curled downward (red arrow).

As you carefully remove the damaged threader hook, notice how it is positioned in the mechanism. The new threader hook will need to go in exactly the same way. The little wire spring (red arrow) is held securely by the threader hook when it is in place on the screw/spindle. Use care that the wire spring does not pop out when removing or replacing the threader hook.

The new threader hook fits under the V shaped point (red arrow) and over the wire spring.

Once you have the new threader hook in place, secure it with the sliding screw and re-attach your wonderful automatic sewing machine needle threader.
