Item comes with box.  Batteries are not included.  It takes 2 AA to run.  Also wanted to note that the battery cover is missing, but the batteries stay in there just fine.

As seen in the pictures, there are some melted marks/scratches.  They are the shape of the cord, so this is either from time sitting against the toy, or something along those lines.  Not sure how they got there exactly, but tried to show the current state in the pictures.
