Dual Secondary ~35/39VCT Red and Red/Y Leads / ~20/21VCT Yellow and Green Leads.  Voltages are Open Circuit (No Load).  Low and high Voltages depend on which Primary Lead you connect Black or Black White.

Suspect this was nominally intended as a 28VCT / ~16VCT Transformer?  Windings apear to be 22AWG on the Red Leads and 24AWG on the Yellow.  Estimating 1A available output per Secondary.

Unknown Current Rating Transformer appears to be at least 20-30VA size overall.

Carson is a California based company that manufactured custom wound transformers.  They have since left this segment of the industry.
