Old School Samsung Verizon Juke Phone. The cover pivots to the side - I have never seen one of these even back when they were being sold! Pretty cool technology for back when they were made in 2007. It reminds me of a secret agent camera!
I don't have the cord so I don't know if it works but there does not appear to be any damage to it so I can't imagine it wouldn't. Comes with black case made specifically for this model.
