Okay, I got that prescription from an older listing.  See the photos for the details - the Motorola adapter picture is the exact one that you will receive. It is still in the original baggie, never been taken out, never been used, it is in new condition.

Shipping is USA only and this will be shipped via USPS Ground Advantage.

Besides based on the total weight, the actual cost of shipping is also based on distance from me here in NY and is automatically calculated when you are logged into your eBay account.
