Book features both issues 4 and 5 and is a unique copy
presenting an extremely rare print error: 10 centerfold pages unstapled.
Only twice in over 30 years I had found unstapled books with loose pages,
but this is the first the stapling is partial. The loose pages are wrongly cut ("out of register")
so that they come out of the closed book.
