I have not tested the cartridge slot as I don't have any games for this device, and therefore returns are free within 30 days if there are any issues, however everything else about the device works perfectly including the software functions so I expect no issues.

The rechargeable batteries are included and hold a charge, however the AC adapter is NOT included (I just used a generic multi-voltage adapter from my personal toolkit). Alternatively, you can run it with 4 size "AA" batteries, not included.

There are a variety of cosmetic scuffs and other signs of use (see photos) but nothing that affects function. The stylus is included but no other accessories.
