You'll receive the exact patch pictured.  Some have speculated that it relates to STS-42 but that's very unlikely the case.  It's more likely an early rejected design for STS-51A that Al Bishop decided to have made into an embroidered patch. 

While it may have been rejected if it was proposed for 51A, the final patch design for that mission nevertheless incorporated similar imagery or thematic features.
