I do not have the original box or packaging materials.  Cisco part number is ASA5505-50-BUN-K9, which allows 50 NAT connections (inside PC's NAT'd out to the internet)  Included is the ASA5505, console cable, AC power supply, and AC power cord.  These are great boxes as two of their LAN ports are PoE ports to power devices like a phone and access point in a home or small remote office.  Fast shipping via USPS first-class package to confirmed addresses in the US only.
