eBay identified it as
"NovAtel Verizon Jetpack 4510L 4G LTE Mobile Hotspot MiFi" but I have no box and assume that is where the NovAtel name might show up?
No box, but unused, still has protective film on modem.
All documentation seems to be included. See photos.
