If quantity is more than one, then exact card may not match the exact condition in the picture, but it will be of similar condition.

Below is how I decided the condition. 
NM - Typically pack fresh, may have a couple white spots on the back. Near Mint does NOT mean mint. 
LP - More white spots maybe some scratches on the holo. 
MP  - Border wear is more apparent. More scratches are apparent and potentially a small creases. 
HP - A lot more wear. Might have liquid damage. At least 70% of the card is still there. Edges will show a lot more wear.
Damaged - Card is missing a lot of the artwork, majorly water damaged, or majorly bent.

Cards under $5 will be shipped between cardboard in an envelope.
Cards 5-20 will be shipped in a top-loader in an envelope.
Cards $20 - 80 Will be shipped first class in a top-loader. Typically a padded envelope.
Cards 80+ Will be shipped in a top-loader in a box.
