This is a 1969 Gillette 23k Gold Super 109 Adjustable double edge safety razor - date code O-4, with original blade bank (and as far as I can tell, the two original blades).

The gold Super Adjustable was marketed in Japan as the New Aristocrat. It was only produced for one and a half years, from January 1969 to mid-1970.
