This item is an Ancient Mayan / Aztec death whistle. 

This Death Whistle is based on an ancient Aztec artifact that makes a scowling scary howling sound. It sounds like a human scream or howl in a dark cave or deep tunnel. The skulls were found in many Mayan graves discovered by the spanish in the 1500s. The whistles were clenched in the deceased’s hands and no one knew what they were at first. A 3D scan of a stone carving of Quetzalcoatl in the National Museum of Anthropology in Mexico City is what this item is based off of. 

The loudness of these can be scary, It has even made my ears ring.
