All above sizes are the Diameter of the Shaft the Ferrule is intended to fit.

Worth noting perhaps, due to the nature of material, 

a 19mm rubber ferrule would fit a 20mm shank for example

and likewise with all sizes. Though the smaller sizes have much less tolerance.

*12mm has fewer rings on the grip than shown.
