It's no wonder the 12614 was a cornerstone of UREI's auditory artistry. Marked  as originals are, on the bobbin, "NYT-10", most likely New York Transformer. A top tier company, long since out of business. 

This specific piece, sequestered in my spare transformer trove for two decades, remains intact, an impressive feat given the fragile nature of these lower plastic bobbins.

Proudly showcasing mounting holes spaced precisely 60mm apart, it's primed to amplify your UREI Compressors, Line amps, or EQ's, elevating your line-level audio output. As for its ancestral roots, pinpointing the exact UREI masterpiece it once served is a bit tricky.
