There are some minor cosmetic issues and it is missing two out of four screws to hold the frame in place.  Still has two screws, so the frame and plque are together.  The frame is still mounted, however the frame does pull way fro plaque.  Quick fix with correct size screws.  Original box does have damge to it.  Please see photos.
