It's double-sided. 

Disc: NM  There are slightly scratches.

Cover: NM  There are  slightly scratches.

Insert : None

Obi: EX There are slightly scratches. The square cut is in the bottom corner of the back.

As for the Disc, I do not have a player, so can not check if it works.
