I believe this was a prototype unit as there’s no markings on the back and the faceplate is mislabeled. The ‘on’ and ‘mute’ work in the same positions as the production pieces, but this faceplate has those labels reversed. 

This preamp was checked out by a tech and found to be working within spec.
