This is a Conrad-Johnson HV-1 Pre-Preamplifier. It is designed to be used with low output moving coil cartridges just as a step up transformer would be used. However, instead of using a transformer, it uses miniature metal triode tubes called Nuvistors. It is in very good condition, there are no visible scratches on  the faceplate or on the black enclosure. It works great and is a fairly rare unit.

I've owned this almost since new. It sounds gorgeous, which is why I've used it for so long, but the signal to noise ratio can't match a solid state unit, and is very dependent on the Nuvistors installed. No hum that I can hear, but there is "tube rush".
 
It must be plugged into a moving magnet phono input.
