You are considering a Quick Shot  Model QS-129N gamepad for the Sega Genesis console. Designed to be held in the left hand with joystick being controlled by right hand and fingers from left hand working the A or B buttons. Works well on Sega Genesis games once your hands get used to the control setup. Has an Auto switch too. Haven't seen any others like it.
