A form specially designed for the Christmas Light Installation business.  Available in a small quantity and a convenient size, An itemized checklist makes filling out your form easy. You can list the services that you provide on the invoice. Form includes space for description of work to be performed and pricing information. Other features such as numbering and a logo can be added at no charge. You can modify this form -  add or delete lines or columns, change headings, etc. what ever you need to make it work for you. We have been designing and printing business forms since 1970 so I will guarantee you will like your form.

SIZE - Set size is 5.5" x 8.5" - Detached size from book is 5.25" x 8".
PAPER- 2 part carbonless paper or 3 part carbonless paper is available.
PAPER SEQUENCE -  2 part - White and canary, 3 part - White, Canary and pink.
INK COLOR - Normally we print these in red and green but other color options are available.
TYPE STYLES - Your choice of any of our Mainline type styles or submit your own. See Ordering Information.
NUMBERING - Numbering is available at no extra charge.
BINDING - Individual sets or bound in books of 50 sets with a hard chipboard backing and a wrap-around  cover.

After you purchase your forms we will contact you for all the details.  If you plan to provide logos and/or artwork, please ensure that the file is a gray scale 300 dpi file in PDF, JPEG or Tiff formats. Screens, gradients and halftones may not reproduce well. If you have a question about the file we will be glad to help you. An electronic proof will be sent to your email address for approval within 24-48 hours excluding weekends and holidays. Any changes in the body of the form may require additional proof time.
