Here is an original script that was used on the set of STAR TREK: DEEP SPACE NINE. 

The script is from the 1994 episode titled "Crossover"

The story: Kira and Bashir find themselves in an alternate universe, and meet very different versions of the people they know from their universe, including a second Kira. This episode is a callback to "The Mirror Universe" episode of the original series. 

The cover is blue and the interior pages are many different colors (white, pink, tan, blue, yellow, green, goldenrod, and bright pink), indicating this episode went through a LOT of revisions. It has 58 pages and is complete. The script was used on set by, I believe, Alexander Siddig as Dr. Bashir. 

In addition to a portion of his name being written at the top of the cover, there's a dialogue note in the script for his character. There are also a couple of "cue" pages written in marker for Nana Visitor to read. There's also bits of tape on some pages, likely used to hang the pages off-camera when Bashir and Kira are in the Runabout, so the actors could read the dialogue if needed. The actors were in a lot of scenes in this episode and Visitor had to play two parts. There are a lot of smudges on the script so it got a lot of use on the set.
