The end of the handle is knurled, has a 3/8 square socket drive, and a transverse hole to accommodate a rod. The knurled portion turns an internal shaft geared to the stud. The physical appearance is similar to a ratchet wrench. The wrench head where a ratchet would normally be found holds a bearing block and beveled gears. The gearing is such that a ratchet-like action can be obtained by swinging the handle while turning knurled end portion. Or a rod, ratchet wrench or breaker bar can be used to "power" the shaft.
