Dayton 5K457 3/4 HP 1 PH 1725 RPM 56 Frame Electric Motor 115/230 V. 

Note there is a little bit of scoring / wear on the shaft end where it looks like a pulley probably came loose at one time.

I powered it up and it runs well.

Used motor it is from a closed factory that kep replacement motors in stock for their equipment there may be some scratches/shelf wear and dust from sitting.
