NOS genuine Ford license lamp lens.Ford superseded this part number several times. Started out as FDM-13564-B, then to B5C-13564-B, then to B7C-13564-B and finally to C1TB-13564-B.
I've seen examples of all of these numbers (on the original boxes), and can find absolutely no difference in them at all - all have the FDM-13564 number molded in, as well as the FoMoCo oval emblem.
These fit 1955 -1963 Ford Stepside Pickup only - do not fit Flareside, nor the 1961 and later Integral Body style.
They may fit other trucks without pickup boxes (like stake beds?), and maybe 1964 and later. Not sure.
While brand new, never installed, the box shown is not included. Ford packaged these more than one to a box, and the box went with the other one that was in the box when I got it. You will be receiving the lens only, nothing more.

A few words to make things easier.
1. Combined shipping for multiple auctions is always available, but when the auction is over, you will have to wait until I revise things in my eBay "paperwork" to create just one invoice for all your purchased auctions. And I have to do the revising BEFORE you pay. If auctions are paid separately (i.e. before you receive my invoice combining items), eBay will no longer then let me combine them on one invoice. AND, eBay then automatically charges you separate shipping charges for each item, which is not refunded. Again, this is eBay's set-up. Combining auctions benefits you by saving you shipping and handling fees.
2. Payment is expected within 3 days. After 8 days, I will file with eBay to have my auction fees refunded. If there is a problem with 3 days, please let me know, as I am not unreasonable, and I know that things happen. Just be aware that having been on eBay full-time for several years has already afforded me an opportunity to hear a lot of "creative" stories.
