Send the item(s) you would like to return or exchange in a secure package via the U.S. Postal Service ONLY. DO NOT USE FEDEX OR UPS as they do not deliver to P.O. Boxes. Please fill out the return section of the form completely and enclose it with the item(s) you are returning.
