Star Wars: The Empire Strikes Back Video Game for the original Intellivision and Intellivision II, Tandyvision and Sears Super Video Arcade. I bought this new and never had time to play it. I just tested It and it works fine when firmly pressed into my Intellivision II. See TV pic. Box is in like new condition! It Contains 1 game cartridge and  playing instructions.
