Inside are pictures of the KYA DJs; Seamus O'Hara, Tony Tremayne, Chris Lane, Lucky Logan & Scotty Day. On the rear is an ad for Stri-Dex medicated pads. #1 song was Alley Oop - Hollywood Argyles. Jammed up at #2, Look For A Star - Deane Hawley. Stuck at #3, Because They're Young - Duane Eddy. Jammed up at #4, I'm Sorry - Brenda Lee. Inching up to #5 from #6, Bill Bailey - Bobby Darin. Slipping to #6 from #5, My Home Town - Paul Anka. Jammed up at #7, Image Of A Girl - Safaris. Jumping to #8 from #12, Mule Skinner Blues - Fendermen. Vaulting to #9 from #23, Finger Poppin' Time - Hank Ballard. Dropping to #12 from #9, Cathy's Clown - Everly Brothers. Jumping to #22 from #28, Train Of Love - Annette. Survey is in good condition; has 3 songs scratched out in black marker; measures 8-1/2 x 5-1/2 when closed.
