From the roots only and 100% raw, untreated and natural.

Long Jack root pieces are used in Malaysia to prepare one's own tea (which basically is a fresh extract from which the water has not been removed).  

Some people also argue that the less processed a natural substance is, the  better its efficacy, and thus prefer chipped root or root powder. 

Long Jack root pieces are not sterile, so the raw consumption is not recommended.

 The traditional preparation in Malaysia (and Indonesia) is to boil chipped roots as a tea. 
 
For best results soak about 5-10 grams of root pieces over night in 1 liter of water (or soak them for a shorter period of time and reuse them!), than boil/simmer the mixture for 15 - 30 minutes in order to extract as much of the active ingredients as possible. 

The longer the mixture will be boiled, the more 'potent' the tea is.

Let the liquid cool down, remove the root pieces and drink.

Caution:

Tea made from yellow Long Jack root can be very bitter. 
 
 
For an alcoholic extract of chipped root, no prior sterilization would be needed. Just insert an small amount of root pieces in a container of alcohol. Best suited  would be a low-price, high percentage vodka.
