Antique 1900s Japan Old Photo Portrait Of Japanese orphans ? nanny nurse Meiji.

Not super sure who we have here. These children appear to be Japanese.  Not sure if photograph was take  in Japan China or USA. Not sure why a white lady has these three children. Could be orphanage or a hospital setting. Little girl has bandaged arm. Little girl also seems to be of mixed race of Japan and Caucasian.?  Funny how all three children are looking at camera and the lady is not.  I'm thinking these are war orphans and the photograph will be used to help find new parents. Quite a bit of curiosity with this shot.  I will send it in its original frame. Consider frame of no value and just as packaging.
