This is the game Ultima IV: Quest of the Avatar by Origin for Commodore 64 computers.  It comes with cloth map, manual, spell book, reference card, metal ankh and an assortment of platform stickers on a piece of paper.  Unfortunately the disks are missing but these should be relatively easy and cheap to find on their own and complete the set!
