Midmark Linear Foot Control for 630 Universal Procedures Table/Chair. May work with similar Midmark models that use an RJ11 style cord (see pics). Footswitch has 8 control buttons and 4 additional function buttons. This unit was previously used in a medical facility and is excellent used condition. All switches respond as expected when clicked. Cord and plug are in good shape with no fraying or broken clips. I do not have the means to test it further but will guarantee this unit will not be DOA.
