My condition system is this:

Mint (M): Games do not receive this rating lightly. The disc has almost no scratches whatsoever and any it may have are light or very short. The case, front insert, manual (where applicable) and any other paperwork/pamphlets the game may have come with are present and will have minimal damage, such as a very slight crease to the manual. The outer sleeve may have very light scratches or very small dents as these sleeves are rather fragile, however, the scratches or dents will not be too deep or too many and there will be no holes of of any kind. Any stickers to the cover will be original from first point of sale and will be visible in the pictures. There will be no stickers on the manual or other paperwork.

Near Mint (NM): The disc will have some small scratches but these will be minor and light. A single deeper scratch may appear if it is no more than 2mm in length. The case, front insert and manual will be present (where applicable), however, extra pamphlets such as warranty cards, advertisements or warnings may be missing. Any larger paperwork such as maps or posters, however, will be present. The case and manual will have very little visible damage outside of the outer sleeve as these sleeves are very fragile, however, the scratches will not be too deep or too many and any holes or dents in the sleeve will be minor. There may be very slight fraying to the top or bottom edge of the front insert, which should be visible in the listing's photos. The manual may have some minor creasing, curving, some small writing or a shop's sticker placed on it.

Very Good (VG): The disc will have many light scratches and possibly a few deeper ones as well. Disc may have been resurfaced. The case and front insert will be present but may have very minor cracks or holes in the sleeve and the manual may not be present, see the title and pictures for a more detailed description. No paperwork is guaranteed to be included, but what is included will be in presentable condition.

Good (G): The disc will have multiple scratches both light and deep, but not so many that the surface of the disc is covered and certainly not to the point where the disc will not play. The case and front insert will be present but may have multiple cracks or holes and the teeth that hold the disc in place may be damaged. The front insert may be frayed or ripped. The paperwork may be included but may also be in very poor condition.

Acceptable: (A): The game will boot to the start screen, however gameplay throughout the game will not have been tested and therefore the game might not load every scenario. The disc will be covered in scratches, most of which will be deep. The case, front inlay and any other paperwork may not be present and if they are there is no guarantee they will be in any condition.
