The unique format for printing is a fold out pamphlet of one piece of paper with a total of 16 small pages each one measuring 5 1/2 inches by 8 1/2 inches . Both pieces are in good original condition as they have been kept in storage with collectibles of a Dr. who was showered with gifts from patients for years.
