It comes with the basket, a mattress and a mattress cover.  The mattress cover is from Gerber and the mattress is from The Simmons Manufacturing Co., LLC - date on the mattress is 7/10/2007 - mattress size is 29" x 14" x 2".

The mattress cover has been washed.

This is a large basket and it is in good condition.
