Men in the study cohort had early-stage disease and were managed with surveillance; they were treated only if there were signs of disease progression. Up to 20 years after diagnosis, 1.5% of the 993 men had died, and 3.1% had developed metastatic disease. In addition, death was 10 times more likely from other causes than from prostate cancer, reported Laurie Klotz, MD, from the Sunnybrook Research Institute in Toronto. "This is the longest, most mature follow-up with the explicit strategy of conservative management and selective delayed intervention," he told Medscape Medical News. "Even at 15 to 20 years, the prostate cancer mortality rate is really very low, you've avoided treatment in the majority of patients, and even the ones treated late still had a long period of normal quality of life before they had treatment." Dr. Klotz presented the results here at the European Association of Urology 29th Annual Congress. At 5, 10,15, and 20 years after diagnosis, 75.7%, 63.5%, 55.0%, and 55.0% of patients, respectively, remain untreated and on active surveillance. During the follow-up period, 15 died from prostate cancer and 7 developed metastatic disease. Results Are Now "Slightly More Sobering" The prospective single-group cohort study enrolled men (median age, 69 years) with histologically confirmed prostate adenocarcinoma who had undergone no previous treatment. Definitive intervention was offered to patients if they had a prostate-specific antigen (PSA) doubling time of less than 3 years, Gleason score progression (to 4 + 3 or greater), or unequivocal evidence (including MRI) of clinical progression. The remaining 993 patients were followed with active surveillance. In an earlier follow-up of the patients, at a median of 6.8 years after enrollment, only 5 subjects had died of prostate cancer ? all of them fairly early into their surveillance (J Clin Oncol. 2010;28:126-131). In all 5, it looked like their disease "wasn't preventable with earlier treatment," said Dr. Klotz. "But now it's slightly more sobering. We're starting to see some deaths in patients who have actually developed their metastases late. Perhaps if they'd been treated a decade before, they might have been cured." "The devil is really going to be in the details about these men who progressed," said session chair Matthew Cooperberg, MD, MPH, associate professor and chair of urology at the University of California, San Francisco. "The question is how many men died of prostate cancer because they went on active surveillance instead of getting immediate treatment. The answer to that question is always going to be greater than 0," Dr. Cooperberg explained. "However, we all think that it's very, very, very low; it's far lower, most likely, than the number of men who are harmed or potentially harmed from overtreatment for low-risk disease." A similar sentiment was expressed by Chris Bangma, MD, PhD, professor and chair of urology at Erasmus University in Rotterdam, the Netherlands. Dr. Bangma is a principal investigator of the ongoing Prostate Cancer Research International: Active Surveillance (PRIAS) study, the largest worldwide prospective active surveillance study. "How do these survival results compare with the curative treatment that patients would have otherwise received if active surveillance did not exist? We do not know yet whether the active surveillance mortality numbers in the study by Dr. Klotz's team are comparable with those in radical prostatectomy studies," Dr. Bangma told Medscape Medical News. However, "it's my expectation that the study numbers will be far below those in the radical prostatectomy series, because the patients have been selected as having very low risk." At this stage, active surveillance "is a treatment approach in evolution," said Dr. Klotz. Research continues into how to refine the parameters for triggering intervention. "We've shifted to a more MRI-based evaluation of patients, which I think in the long run is going to change the shape of the time-to-treatment curve," he said. "When something doesn't seem right ? either a patient has a short [PSA] doubling time or some small amount of Gleason pattern. Scientists have sequenced the genome of the world's oldest continuously surviving cancer, a transmissible genital cancer that affects dogs. This cancer, which causes grotesque genital tumors in dogs around the world, first arose in a single dog that lived about 11,000 years ago. The cancer survived after the death of this dog by the transfer of its cancer cells to other dogs during mating. The genome of this 11,000-year-old cancer carries about two million mutations -- many more mutations than are found in most human cancers, the majority of which have between 1,000 and 5,000 mutations. The team used one type of mutation, known to accumulate steadily over time as a "molecular clock," to estimate that the cancer first arose 11,000 years ago. The genome of this remarkable long-lived cancer has demonstrated that, given the right conditions, cancers can continue to survive for more than 10,000 years despite the accumulation of millions of mutations," says Dr Elizabeth Murchison, first author from the Wellcome Trust Sanger Institute and the University of Cambridge. The genome of the transmissible dog cancer still harbors the genetic variants of the individual dog that first gave rise to the cancer 11,000 years ago. Analysis of these genetic variants revealed that this dog may have resembled an Alaskan Malamute or Husky. It probably had a short, straight coat that was colored either grey/brown or black. Its genetic sequence could not determine if this dog was a male or a female, but did indicate that it was a relatively inbred individual. We do not know why this particular individual gave rise to a transmissible cancer," says Dr Murchison, "But it is fascinating to look back in time and reconstruct the identity of this ancient dog whose genome is still alive today in the cells of the cancer that it spawned." Transmissible dog cancer is a common disease found in dogs around the world today. The genome sequence has helped scientists to further understand how this disease has spread. The patterns of genetic variants in tumors from different continents suggested that the cancer existed in one isolated population of dogs for most of its history," says Dr Murchison. "It spread around the world within the last 500 years, possibly carried by dogs accompanying seafarers on their global explorations during the dawn of the age of exploration." Transmissible cancers are extremely rare in nature. Cancers, in humans and animals, arise when a single cell in the body acquires mutations that cause it to produce more copies of itself. Cancer cells often spread to different parts of the body in a process known as metastasis. However, it is very rare for cancer cells to leave the bodies of their original hosts and to spread to other individuals. Apart from the dog transmissible cancer, the only other known naturally occurring transmissible cancer is an aggressive transmissible facial cancer in Tasmanian devils that is spread by biting. "The genome of the transmissible dog cancer will help us to understand the processes that allow cancers to become transmissible," says Professor Sir Mike Stratton, senior author and Director of the Sanger Institute. "Although transmissible cancers are very rare, we should be prepared in case such a disease emerged in humans or other animals. Furthermore, studying the evolution of this ancient cancer can help us to understand factors driving cancer evolution more generally." Last week, U.S. News ran a rather sensational cover story on teen-age sex, making what I think is the mistake of treating teen-age sex as something novel or unnatural. Teen-agers have been having sex forever. Their bodies are maturing, their hormones are raging and doing what comes naturally is, well, natural. Indeed, for most of human history, teen sex was an entirely normal part of life, since people tended to marry and be treated as adults at what were, by modern standards, very early ages. Programs that don't take that into account aren't likely to succeed, and media coverage based on sensationalism doesn't help. But as blogger Moira Breen observed: For a lot of teen-agers, teen sex isn't about sex, or at least enjoyable sex, but about the status of having a boyfriend or girlfriend. That's no surprise, given that teen-agers often have nothing else. Once, teen-agers weren't a demographic: They were adults-in-training. They worked, did farm chores, watched children, and generally functioned in the real world. They got status and recognition for doing these things well, and they got shame and disapproval for doing them badly. But since sometime in the mid-20th century, the role of teen-agers has been different, as a recent article by Thomas Hine in American Heritage magazine points out: Young people became teen-agers because we had nothing better for them to do. High schools became custodial institutions for the young. We stopped expecting young people to be productive members of the society and began to think of them as gullible consumers. We defined maturity primarily in terms of being permitted adult vices, and then were surprised when teen-agers drank, smoked, or had promiscuous sex. No longer adults-in-training, teen-agers became part of their own social class, on the one hand indulged and sheltered, on the other viewed as juvenile, oversexed and somehow dangerous. In fact, far fewer teen-agers were shouldering adult responsibilities. Instead, they had a new, increasingly cosseted role as high school students, social butterflies (and outcasts) and consumers. Increased sexual activity, research indicated, was directly related to increased schooling and decreased responsibility. Teen-agers may be busy with teen activities, but not with adult responsibilities, and it shows. Hine is right. We have infantilized teen-agers, and then we act surprised that they behave immaturely. And lately we seem even to be infantilizing 20-somethings: Reports on "Taliban" John Walker Lindh, or Monica Lewinsky, or Louise Woodward, the nanny accused of killing her charge, all tend to feature references to their alleged youth, though at their age historical figures like George Washington or Teddy Roosevelt had been acting as adults for years. Unfortunately, the most common response we see to reports of teen-age sex is to further infantilize teens. This serves the interests of nanny-state advocates who want to infantilize everyone, and of those who believe that coercion is the only path to virtue. When this approach fails, as it is sure to do, that will justify still more coercion. (Abstinence programs, the U.S. News story reports, make teens more anxious to retain "technical" virginity, but often via riskier practices like anal sex.) I recommend a different approach: If we want teen-agers to be more adult, in their virtues as well as their vices, we should try treating them more like adults. Teen-agers should be encouraged to hold jobs in addition to going to school. (Or instead of since high school is not for everyone.) It may seem odd to argue that dropping out and getting a job might be a good thing for some teen-agers, given the high value we are supposed to attach to education. But education only matters when people are being educated, and for all too many students high school isn't really about education anyway. Much of high school is wasted time: School meets only about 180 days a year, with a lot of class time wasted on going over the same ground from one year to the next. Teen-agers with a powerful desire to be adults should be allowed to follow an accelerated program, with earlier graduation (and perhaps other privileges) as a reward. Many teen-agers would take advantage of this, rather than spending extra years in what's little more than a pre-adult holding tank. Today's high-schoolers do work, but in general they're far less responsible for their own support, and far more separated from adult responsibilities and achievements, than people their age have ever been. Where once it was possible to gain status through actual accomplishment, now the chief status accessory is a boyfriend or girlfriend. Shorn of opportunities to exercise adult virtues, teens work with what they've got. Consider this analogy: Unmotivated teen-agers who are idling away their time in school, protected from the real world and supported by their parents, are more like welfare recipients than they are like responsible citizens. However, since the implementation of welfare reform has forced a degree of personal responsibility, illegitimacy rates are way down, and so are many other social pathologies associated with welfare dependency. Maybe what teen-agers need is some "welfare reform." Perhaps if teen-agers were encouraged to take on adult responsibilities and win status and recognition in constructive ways, they'd probably start acting more like citizens, and less like a leisure class, with all the vices that have historically attended leisure classes. If teen-agers weren't infantilized in so many other ways, they'd have a better base of judgment and self-respect, and could make better decisions about when they were ready to have sex and be more responsi

                                        e about precautions and consequences when the time came. Of course, lots of people would lose under an arrangement like this. Marketers would lose their grip on a key demographic. Busybodies would lose an important opportunity for tut-tutting. Journalists would lose an opportunity to run articles on teen sex that are simultaneously censorious and exploitative. And politicians would lose an important opportunity for posturing.
