This very hard to find, vintage 2003 gift shop item was from the National Cathedral in Washington DC. This comes with the original tag in a presentation gift box that commemorates the Cathedral's 100 year anniversary.

This measures 6.5 on one side, 10.5 on the other, and some of the cord is between the layers of the tag.
