From Solon, Ohio and then box has a stamp of Ravenna (nearby city). 

Box was unscrewed to show you the inside. 

Total weight is 27 Lbs. 

All push knobs and switches feel like they are still connected and move how they should. 

It appears unit was used laying down and top handle was towards them since the dial and tag markings are all upside down when standing up tall.
