I grade my

EX-Totally
unmarked, plays great with no sticks/jumps & very little background noise.
VG-Some
light surface marks visible, plays with no sticks/jumps, some background noise
may be heard, mainly between tracks or on some quieter parts.
GD-More
pronounced surface marks, plays with no sticks/jumps, surface noise may be more
evident with occasional pops/rotation sounds etc, nothing major.
Fair-
Surface marks highly visible, plays with no sticks/jumps, but background noise
may some-time get in the way of the music.
Poor- Record
will have at least one jumps/skip.
I always
test play & give a brief description of how each record plays.
