The medal is made of light bronze. Diameter 28 mm. On the obverse of the medal there are two bust portraits: Emperor Nicholas II in the uniform of a colonel of the 4th Infantry Imperial Family of the Life Guards Regiment and the tsar, the founder of the dynasty, Mikhail Fedorovich wearing a Monomakh cap and barmas. Along the edge of the medal there is an ornament of repeating lines and dots. The image on the obverse is very close to the image on the 1 ruble commemorative silver coins. On the reverse side there was a horizontal inscription in five lines [1] [4] [6]:

The main circulation (more than 1,500,000 copies) was produced at the St. Petersburg Mint. In total, by June 26, 1914, the mint received at least 2,028,166 orders for the production of medals [2]. The author of the project of the medal is Anton Fedorovich Vasyutinsky [2]. The author of the bas-relief on the obverse of the medal is the sculptor Mikhail Arkadievich Kerzin [9].

The production of medals by private workshops was allowed. They could differ significantly in image details. Some medals made by private workshops were of silver [10], sometimes gilded, and, less often, gold [2] [11]. In addition, tailor-made versions of the medal are known, with a diameter of about 15-16 mm.
