There is a piece cut out of page 5 It measures approximately 8 1/4 inches by 10 3/4 inches.  It was published by Delta Magazine Inc.  copyright 1963. Printed in USA. 

The front cover is in color with the back cover in two tone and the interior pages of the magazine black and white.
