Case is sold as pictured.  Please note that it was made for M42 screw mount lenses, it will not fit Pentax 135mm K mount lens. That newer lens is little bit larger and thicker. 

This case was made for Pentax 135mm f3.5 Silver lens, with 46mm filter. It is oldest type. Case is sold as pictured, all pictures are of an actual item for sale.
