Barcus-Berry 1330 preamp. This preamp is used with piezo pickups to record acoustic instruments (acoustic guitar, violin, etc!). According to gear sites, the 1330 model was used by Jimmy Page for recording acoustic guitar.

Has been a standard for pro players since the early 80's or earlier!

Uses 9 Volt battery (not included).
