Schuremed Replacement Headrest assembly 790-0063-M Double Ball Joint Assembly with Malleable insert.  

I believe this may have been a new replacement that was never installed, excellent condition, no scratches or marks, pad is clean, and includes one set of disposable straps for head positioner.  

Will fit any Schuremed manufactured by Schuremed, including Skytron, Berchtold, Steris models.
