I am identifying her as a transitional #4 doll. Her face, arms and legs are of a #4, but the torso is hollow and has an "R" on the behind.

As to the condition, she has no neck splits; but one very light green ear (right) which she was treated for, and it's now hardly seen. She also has a super tiny nose nip which I only noticed when I zoomed into the photos after I took them. Finally, her face, arms and legs are more yellow than the torso. Her legs are not too tight; they are a bit swingy.
