A great original Treasury Department issued order form for Opium, Coca Leaves, or Compounds, Manufactures, Salts, Derivatives or Preparations! A similar form was recently shown on the History Channel during a program on illegal drugs. The Treasury Department used a certain dye to make this form look old. Please see photos!


The form is ornate with a beautiful poppy graphic on the top corner. It was issued to Pharmacist, E. Mae Beales of People's Drug Store, in Gettysburg, PA. An order for two types of Morphine was placed on March 13, 1944.

 
This red duplicate was kept by the doctor/pharmacist for his/her records, the top original order form in black, was sent to Hensel & Sons in Harrisburg, PA, to be filled.



The form is large, 10 3/4" by 8 1/2", has a round punch mark on the top, two carbon rubs top left and light bends, otherwise, condition is excellent, a beautiful form...a personal favorite!
