Lens SN 377854. Lens has universal M42 mount. And it has removable T2 m42 mount adapter. 

Lens looks OK.  Aperture works good. It opens and closes. Please note that blades have some oil, but since it is manual aperture , it has no affect on lenses performance. Blades are not sticky. Focus works well. Glass looks very nice. I don't see any fungus or haze inside. it has couple very tiny dust specs inside. And i see one large bubble in the front. That is manufacturer defect. But over all lens works and looks good.

Lens makes very nice bokeh, but it is soft wide open, it has low contrast. And it flares easily, so you have to get lens hood for that.
